Medical Requirements Any new student to CPS or children in K, 5th, and 8th grade must have a new physical on file by October 15th.
On the first day of school Registration Will be processed as soon as possible. We hope to begin at 9:30 or 9:45 a.m.

Birth certificate or other proof of age, immunization records, physical and dental medical records
2 proofs of Current Address:
Illinois ID/License Utility Bills Court Documents (Call school if you have questions).
